  • Important Notes for Emergency Relief Applications
    1. Please read the application guidelines of the Chang Yung-Fa Charity Foundation’s Social Relief Application Rules and Regulations first.
    2. Please download and print the application form directly from our website, or call our regional offices to receive the application form by fax.
    3. Please fill in all required fields of the application form.
    4. Applicants and referral units must sign or affix seal on the relevant fields in the application form.
    5. Please attach the necessary documents to the application form and submit them to the Foundation for review.
    6. When the application form and related documents are ready, please submit them to the closest branch offices of the Foundation according to your residing county or city.
    7. We will process case applications in the order of receipt. Please wait for our staff to contact you by phone.

Q & A for Emergency Relief Fund

  • I am encountering difficulties, do I need to apply directly to the Foundation?

    Sorry, we do not accept applications directly from individuals. Please apply through referral units, such as village/borough chiefs, social workers from local district offices, police officers, etc. (referral units are listed in the Application Rules and Regulations). Please make sure to apply for assistance through the referral unit and check whether the official seal of the referral unit is affixed in the second page of the application form.

  • What is a referral unit?

    In the list of assistance measures provided for in the Foundation’s Social Relief Application Rules and Regulations, among the units recognized by the Foundation, please choose a unit that you feel familiar with your family situation and is willing to assist you in endorsing the second page of the application form as a referral unit. That is, to assist you in referring your case to our organization for assistance assessment, such as village/borough chiefs, social welfare departments of public offices, government-registered social welfare foundations, counseling offices of various schools, social work offices of hospitals, police stations, etc. (Please refer to the Application Rules and Regulations for details of referral units).

  • How do I get the application form and how do I fill it out?

    Method 1: Please download and print the application form directly from the official website of the Chang Yung-Fa Charity Foundation (https://www.cyff-charity.org.tw/).
    Method 2: Receive via fax. Please call the Foundation’s branch office closest to your location and ask for the fax number. If you do not have a fax machine, you can go to a convenience store and provide the code of the fax machine to receive a fax from our staff.

  • What documents do I need to prepare to apply for assistance?

    ​To prove the authenticity of your circumstances, the Application Rules and Regulations and the application forms specify what documents are required to be attached. Please read the Application Rules and Regulations carefully.

  • How do I send the application form and supporting documents after they have been affixed w

    Please send it by registered mail to the branch office of the Foundation closest to you.

  • Will the relief fund be granted upon application?

    Submitting an application does not guarantee automatic approval of financial assistance. The applicant must still undergo an evaluation process by our organization. We conduct a written review, phone interview, and home visit if necessary. We may also verify information with referral or other relevant agencies regarding the applicant's household situation. After evaluation, it is possible that we may not provide assistance or return the application, or we may offer appropriate assistance based on the level of hardship. The assistance rate is not determined to be 100%, so applicants are advised to read the Application Rules and Regulations guidelines carefully and ensure there are no issues before submitting an application.

  • How long does it take to get the review result after sending the application?

    After receiving the application, the processing time may vary depending on the amount of cases received previously. Generally, it takes about 3-4 weeks. According to the Emergency Relief Rules and Regulations, applicants who have been approved for assistance will be notified. However, if the application is not approved, the applicant will not be notified, only the referring agency will be notified.

  • How much will the subsidy/relief fund be?

    There is no set amount of subsidy or support package, and the granted amount shall vary depending on a case-by-case basis.

  • How will the subsidy/relief fund be provided?

    For approved applicants, funds are usually provided by remittance, but cash will be delivered to the applicant in person in case of emergency or account problems.
